Welcome to CLSU Testing and Evaluation Center Online Application for College Admission 2026-2027

The Online Application for CLSU College Admission starts with reading the general guidelines and procedure. Make sure that you have read and understood all instructions.

CLSU CAT 2026 Application Procedure

Step 1. Create an Account

1. Select Applicant Category:
   1. New / Beginning First Year
   2. Transferee*
*NOTE: For Transferees, you will be asked to upload your Transcript of Records (TOR). The CTEC will evaluate your credentials and will send you further instructions through email.
2. Fill in your LAST NAME, FIRST NAME, MIDDLE NAME, and NAME EXTENSION (if any).
3. Please provide your current and active personal Gmail address and request an OTP (One Time Password). Enter the 6-digit OTP that has been sent to your email. You may check your SPAM folder if not found in your inbox.
4. Indicate your date of birth.
5. Create and confirm your password (at least 8 characters). Remember this password, as you will use it every time you sign in to the portal. Please note that this password is not the same as the one for your Gmail account
6. Click SUBMIT to complete the process.

Step 2. Fill Out the Online Application Form

Make sure to supply all required information. There are 3 parts to be completed which include:

1. Course Choice
2. Personal Information*
3. Educational Background

*NOTE: In this part, you will be asked to upload an electronic copy of your recent picture (2x2 colored, studio-taken, white background, with nametag: SURNAME, FIRST NAME, MIDDLEINITIAL, file size less than 2MB)

Step 3. Fill Out Other Relevant Information

Make sure to supply all information.

Step 4. Upload Documentary Requirements Saved as PDF Files

These include:

1. Scanned copy of PSA/NSO birth certificate;
2. Scanned copy of SF-10 or Form 137-A (Junior High School, Grade 7-10), reflecting the grade point average (GPA) in each grade level, and duly signed by the school principal or school registrar*;
3. Scanned copy of SF-9 or Form 138 (Grade 11), reflecting the grade point average (GPA) in each semester, and duly signed by the school principal or school registrar*

*NOTE: For Alternative Learning System (ALS) graduates - Certificate of Rating/Equivalency issued by the Bureau of Education Assessment (see sample document)

REMINDER: It is recommended that you name the file appropriately to avoid incorrect/improper uploading.

*NOTE: If the document is more than 2 pages, merge them into a single PDF file.

Step 5. Download and Print the Application Form and Test Permit

After printing the application form, AFFIX YOUR SIGNATURE ABOVE YOUR NAME. Ask your parent/ guardian to do the same. Do not forget to indicate the date. The test permit includes information on the venue, day, and time of the examination. It also includes important reminders, so read it carefully.

IMPORTANT: Please make sure that the general averages indicated on your application form are VERIFIED AND CERTIFIED by having YOUR SCHOOL PRINCIPAL or REGISTRAR sign the section for Scholastic Records.

You will bring the ❶ application form duly signed by the applicant, parent or guardian, registrar or school principal ❷ test permit, and ❸ school ID or any valid ID with picture.
