The Online Application for CLSU College Admission starts with reading the general guidelines and procedure. Make sure that you have read and understood all instructions.

*NOTE: In this part, you will be asked to upload an electronic copy of your recent picture (2x2 colored, studio-taken, white background, with nametag: SURNAME, FIRST NAME, MIDDLEINITIAL, file size less than 2MB)

REMINDER: It is recommended that you name the file appropriately to avoid incorrect/improper uploading.

After printing the application form, AFFIX YOUR SIGNATURE ABOVE YOUR NAME. Ask your parent/ guardian to do the same. Do not forget to indicate the date. The test permit includes information on the venue, day, and time of the examination. It also includes important reminders, so read it carefully.
IMPORTANT: Please make sure that the general averages indicated on your application form are VERIFIED AND CERTIFIED by having YOUR SCHOOL PRINCIPAL or REGISTRAR sign the section for Scholastic Records.
You will bring the ❶ application form duly signed by the applicant, parent or guardian, registrar or school principal ❷ test permit, and ❸ school ID or any valid ID with picture.
